We are now expanding our rehabilitation services and commit to build up a strong professional team to work for the Pneumoconiosis and Mesothelioma patients in the community. We promote an individualized life-long rehabilitation process and adopt case management approach in our programmes.

 

Key Responsibilities

  • Act as the Board’s ambassador for providing a first line contact point between patients and caregiverswith the Board and promoting the Board's services to a cohort of around 1,450 patients.
  • Provide home visits and hospital visits to patients for understanding their needs and assist them in solving various problems such as usage of medical appliances funded by the Board.
  • Work closely with staff of outsourcing medical teams, exercise centres and NGOs to monitor patients' health status and rehabilitation progresses.
  • Facilitate healthy and active life style, promote healthy ageing and empower patients and caregivers.
  • Assist patients in applying for compensation, rehabilitation aids and other subsidies either from the Board or other sources.
  • Prepare various patient's reports and maintain proper records in the Case Management System ("CMS").
  • Other duties assigned by the management deemed appropriate.

Requirements

  • A registered nurse with at least 6 years of post-registration working experience in medical/ geriatric/ adult respiratory/ rehabilitation field.
  • A recognized Bachelor degree in Nursing is a must with valid practising certificate issued by Hong Kong Nursing Council.
  • Relevant training in counselling is an advantage.
  • Experience in working with elderly and chronic disease patients as well as with multi-disciplinary team for providing rehabilitation services and in conducting outreach programmes.
  • Frequent outdoor works and occasional works outside office hours will be required.
  • Contract Case Manager will have 15% of total basic salary as end-of-contract gratuity (after deducting the contribution of Mandatory Provident Fund by the Board) while Part Time Case Manager will be paid on a pro rata basis with reference to a Full Time Case Manager.
